I play on a newer AMD multicore and was bottlenecking hard on physics/clutter stuff. Alien. I've tried every single one of these fixes, plus a bunch more not listed here and not a single thing has worked, or even made the game playable at all. I'd advise getting one for each fan vent on your laptop.
I have a slightly less powerful rig, but I'm running it at 60 FPS with V-Sync on. Being a GTX 570 though, there's always the chance that the card was on its last legs — although this is a bit of a horrific way for a fine card I don't know how I feel about this, maybe it will work.. I'm somewhat less concerned but no less confused. I believe your performance issue isn't specific to Diablo 3. And then some of those have been downright problematic, with users complaining that new releases were responsible for crashes or blue screens of death (BSODs).
When it was crashing I was using Nvidia Graphics Driver Version 376.19 the latest game ready driver from Nivida. I have also disabled the foreground FPS limiter. When I chose the optimize tick, the game ran horribly, entering a game with 4 players was a no-go, even sometimes in rifts with heavy monster density the game just ... I HAVE ALSO TRIED VERSION 280 WITH PRE-RENDERED FRAMES TO 0 AND DIDN'T NOTICE A MAJOR DIFFERENCE.
But worked great for me, and has good reviews. But things have been prickly of late, and 2016 has been marked by a string of spotty releases that have led to some frustrating bugs for players. Never had any issues with GFE either *knock on wood*. Saw a protip about how you can hit control-R to see frame rate, tried it and shat a brick. A lot of it is the background processes. This made me realize there are two types of performance issues here.
I've lowered the lightning to Low. I don't own AMD (ATI) products so I don't really know what they call their drivers. The nice thing about this is now you're softening the image physically and not through your system. Both of my PCs are fine and my wife's is fine as well.

So perhaps some of things I have gathered here will help us out to least get the game to a playable, not-so-frustrating state until (HOPEFULLY) there is a real fix. 1) There is alot of screen tearing and black bars, and the game sometimes completely freezes except for the sound and mouse.

I'm not ruling anything out at this point. However I did read that someone has an issue with ALL blizzard games, so i ran hearthstone and heroes which BOTH have the same issue.
